How do I create a performance review form where the manager and employee both esign it (so will need to be able to enter the managers or employees email addresses at the time of the review and once final send to a HR email? (needs to be able to enter difference email addresses as the form is completed)

- Add an Approve & Sign element to your flow for the first signer (for example, the employee).
- Click the gear icon on the Approve & Sign element to open its properties.
- Under the outcomes, choose which outcomes require a signature (for example, the approval path).
- In the Signers section, enter the signer’s email or, to make it dynamic, select Form Fields and choose the employee email field from your form.
Once that is set, the employee will receive an approval/sign email and must provide their signature when they approve or deny, depending on your settings.
To include the manager as a second signer, you can add another Approve & Sign element after the first one and set its Signers field using the manager email form field via Form Fields. This way, both emails are taken from what is entered on the form at the time of the review.
Phase 4: Send the final result to HR (fixed or dynamic email)
To send a final report or summary to HR after both signatures are completed, you can use the Flow Report element in the workflow.
- Add a Flow Report element at the end of your approval/sign flow (after both Approve & Sign elements).
- Click its gear icon to open properties.
- In the properties panel, enter your HR email address as the recipient. If you ever need this to be dynamic, you can also use Form Fields to pull an email from the form.
- Select Settings and Edit to customize the PDF and email report that HR will receive.
- Complete the rest of your flow so that the Flow Report is triggered only when the process is finished.
HR will then receive an email report of the approval process, including who approved/denied and any comments, giving them a clear record of the signed review.
